Job Summary

Under general supervision, the professional nurse adheres to the American Nurses Association (ANA) standards of professional nursing and performance, and the ANA Code of Ethics to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes. Utilizes the Theory of Human Caring and Caritas in daily practice. "Jean Watson, PhD, RN, AHN-BC, FAAN, Founder / Director, Watson Caring Science Institute."

This role is eligible for shift differential when the majority of the hours worked fall in a night, weekend or evening shift.

Responsibilities

---Maintain accurate, detailed reports and records---Monitor, record and report symptoms and changes in patient's condition and vital signs---Develop and modify patient care plans as indicated by patient's responses and conditions---Modify patient treatment plans as indicated by patient's responses and conditions---Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care---Monitor all aspects of patient care, including diet and physical activity---Direct and supervise less skilled nursing/health care personnel---Prepare patients for, and assist with examinations and treatments---Assess the needs of individuals, families and/or communities to identify potential health or safety problems---Instruct individuals and families on health education and disease prevention and after care---Administer nursing care to ill, injured, convalescent or disabled patients---Medication administration, documentation and reassessment---Promote positive patient experience---Other job duties as assigned per unit addendum

--- Assessment - Collects comprehensive data pertinent to the healthcare consumer's health and/or situation.

oDemonstrates an understanding of assessment findings as they relate to disease process and continuum of care.

oRelays findings to interprofessional healthcare team.

oIdentifies and reports any abnormalities in assessment data per healthcare consumer specific parameters.

oPerforms data best practices and HIPAA compliance.

oCollects data to promote healthcare consumer, self, unit, and hospital improvement.

--- Diagnosis - Analyzes the assessment data to determine the nursing diagnosis and the pertinent healthcare consumers' barriers to health.

--- Outcomes Identification - Partners with healthcare consumer and interprofessional team to identify expected outcomes.

--- Plan - Partners with healthcare consumer and interprofessional team to develop a plan with measureable outcomes and goals that prescribes strategies and alternatives to attain expected outcomes.

oInitiates a unique, outcome focused plan of care based on current nursing practice.Coordinates safe discharge planning process and follow-up care.

oAssesses healthcare consumer needs for continuing care and implements a plan that matches these needs with the available and appropriate resources through an interprofessional approach.

--- Implementation - Implements the identified plan by coordinating evidence-based care delivery and employing strategies to promote health and a safe environment.

oImplements care based on prioritized healthcare consumer needs.

oPartners with the healthcare consumer, family, significant others, and care givers as appropriate to implement the plan in a safe, realistic, and timely manner.

--- Evaluation - Evaluates, in partnership with the healthcare consumer, the effectiveness of the planned strategies in relation to the healthcare consumer's responses and attainment of expected outcomes.

oParticipates in assessing and assuring the responsible and appropriate use of interventions in order to minimize unwarranted or unwanted treatment and healthcare consumer suffering.

--- Ethics - Practices ethically and utilizes the ANA Code of Ethics to guide their decision-making.

oDelivers care in a nonjudgmental, nondiscriminatory manner that preserves the healthcare consumers' rights and confidentiality for the continuum of care from birth to end of life.

oCreates and maintains a secure, trusting environment by not sharing information learned on the job, except when necessary in the performance of the job responsibilities or to improve a healthcare consumer's care.

oActs as a healthcare consumer advocate.

--- Education - Obtains knowledge and competence that reflects current professional nursing practice.

Knowledge, Skills & Abilities

---Adherence to Standards of Professional Nursing Practice/ Process / Performance [ANA Standards addendum- signed] and accrediting body standards.---Ensures all duties, responsibilities and competencies are conducted in a manner that is effective and appropriate to patients/clients to who care/service is being provided. ---Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient's status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided.---Employee has completed and met their clinical competency standards---Ability to use medical equipment and instruments to administer patient care.---Skilled in maintaining effective working relationship with patients, employees, and the public.---Ability to react calmly and effectively in emergency situations---Ability to interpret adapt and apply guidelines and procedures---Ability to communicate clearly.Computers and Technology---Microsoft Office Suite required.---EPIC documentation preferred.

Education

--- Associate's Degree (Required) Completion of an accredited nursing education program that is accredited and satisfies the licensing requirements of the Colorado State Board of Nursing for Registered Nurses. Bachelor's degree in Nursing (BSN) preferred. Experience: At least one-year RN experience in a direct patient care setting required.
