PACE Home Health Division
M-F 8a-5pm JOB SUMMARY Under the direction of the Center Director, works with the Home Care Coordinator to provide home health assessments to meet regulatory requirements per State guidelines. Makes visits to participant homes to assess home care needs. Performs initial assessment, annual reassessment and change of condition assessments of participants in their home environment and coordinates plan of care with appropriate resources. When appropriate provides treatments and health education for participants in their home as appropriate. Ensures compliance with competency requirements. Maintains the participant's medical record with thorough documentation. Communicates participant's changes and progress to appropriate staff. Collaborates with Home Care Coordinator to create a plan of care for participants. EDUCATION AND QUALIFICATIONS

  • Must possess a current RN license in the state of Colorado; BSN preferred
  • Must have a minimum of one year RN experience working with the frail or elderly population
  • Must successfully perform position specific competencies for RN prior to assuming participant care
    • Must successfully pass agency required background checks and drug screening
      • Must possess a valid driver's license, provide proof of insurance (a copy of your "Declarations Page" indicating you have $100k/$300k Bodily Injury/$100K Property Damage coverage) and have reliable transportation
        • Must comply with PACE health screening and immunization requirements including medical clearance for communicable diseases and up-to-date immunizations.
        • Must possess current BLS/CPR certification at time of hire
        • Must be able to demonstrate computer competency
        • Broad knowledge base of health care problems of the frail elderly
        • Knowledge of common safety hazards in the home and improvements to establish a safer environment
        • Skilled in identifying problems and recommending solutions
        • Knowledge of home care services, nursing principles and practices with particular reference to the elderly
        • Skilled in physical assessment and triaging of medical conditions and based on findings, develop a plan of care
        • Knowledge of health counseling techniques and practices
        • Skilled in preparing and maintaining records and written reports and responding to correspondence
        • Excellent organization and documentation skills
        • Familiarity with community health resources, which are available to participants and families
        • Skilled in developing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers
        • Ability to react calmly and effectively in emergency situations ESSENTIAL DUTIES
          • Assure development and maintenance of nursing standards, policies, and procedures based on established nursing knowledge and research in accordance with all applicable federal, state, local professional and regulatory requirements.
          • Participate in the on-call process. Communicate with IDT team as appropriate.
          • Respond to and perform triage of incoming telephone calls.
          • Communicate effectively with the PACE Medical Director and the Center Director to keep them abreast of any issues or concerns.
          • Oversee and or perform initial assessments of participants in their home environment to determine their individual needs and coordinate plan of care with caregiving resources. Coordinate with the interdisciplinary team to develop care plan for each participant.
          • Oversee or perform annual re-assessments of all participants to determine that appropriate in-home services are provided and that participants are in the least restrictive environment.
          • Follow all PACE Program policies and procedures and OSHA safety guidelines.
          • Handle potentially infectious specimens with appropriate biohazard precautions and practice standard precautions.
          • Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
          • Act only within the scope of his or her authority to practice. WORKING CONDITIONS AND PHYSICAL DEMANDS
            • Active position with moderate physical demands, but must be able to lift up to 50 pounds and assist with patient transfers
            • Must be able to see and hear monitors, alarms, lights and other workplace environmental items.
              • May be exposed to infectious diseases
              • May be exposed to a variety of patient care settings and conditions
              • Requires frequent driving that may include inclement weather, road conditions, darkness, etc. within service area
