Voted by employees "Top Workplaces" by The Denver Post Full Time / Day Shift / M-F - 8 AM - 5 PM w/ one weekend on-call shift per month POSITION SUMMARY: The Certified Nurse Aide (CNA) performs various services within their scope of practice for patients as necessary to meet personal needs and to promote comfort and dignity in any hospice care setting. Responds to inquiries or requests for information and reports any complaints, problems and concerns to supervisor. Promotes positive public relations with patients, family and facility staff. KEY REPORTING RELATIONSHIPS: Reports pertinent information to the Team Leader and Case Manager.Position may receive supervision and delegation from the Registered Nurse or Clinical Team Director (CTD). ESSENTIAL DUTIES: Plans with team manager and primary care nurse for patient/family needs and coordinates patient scheduling needs with the assigned Clinical Operations Assistant (COA). Assists with ambulation, range of motion exercises, positioning/turning, transfers according to patient's abilities, tolerance and preference. Provides personal care to patients including: bathing, peri-care, shampoo, oral hygiene, denture care, nail care, shaving, and assisting to dress as assigned and appropriate. Assists with patient's toileting needs including bedpan/urinal use, providing catheter care, changing briefs, and when delegated and supervised, administers suppositories/enemas. Addresses food and fluid issues by preparing meals, feeding patient, offering fluids and positioning patient appropriately. Maintains patient's skin integrity to prevent pressure ulcers, skin tears, or other damage by changing incontinent patients, turning, re-positioning and applying lotion. Provides timely and accurate documentation according to assignment and patient acuity in relation to plan of care using electronic medical records (EMR). Functions effectively with assigned technology. Participates in In-services and meetings as requested by Team Leader or CTD to ensure up-to-date knowledge of issues and practices. Other duties and responsibilities as assigned. CORE COMPETENCIES: Supports the mission and values of hospice. Knowledge of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of patients in a safe, secure environment. Follows infection control standards, including appropriate use of disposable gloves and proper disposal of biohazard waste. Communicates pertinent patient/family information to team. Maintains confidentiality of all patients/families by adhering to HIPAA standards. Completes continuing education (CEU's) as required by State Regulations. Cooperates with all co-workers and demonstrates flexibility, dependability, professionalism and maturity to perform other duties as assigned. Maintains and respects professional and personal boundaries. POSITION QUALIFICATIONS: Minimum Education: High School diploma or equivalent Minimum Experience: 3-6 months experience (hospice preferred) Required License: Current CNA licensure in the state of Colorado. Valid Colorado state driver's license and automobile insurance.
