This involves administering formal and informal programs within the scope of practice of this classification based on in-service training and providing for and teaching self-help skills with a training approach. The position interacts with individuals in developing social skills, recreational activities and transportation requirements. This position is involved with medical emergencies and behavioral challenges, and other duties as assigned. Duties required may include, but not limited to: Implementation of Individual Program Plan * Transports (drives) individuals to their medical appointments and/or activities outside the facility of Homelake. * Protects the rights of persons served and enhances their quality of life. ADL Skills * Under the direction of the Activities Director and within the scope of training provided: performs or assists residents on all activities. This includes but is not limited to: One on One's, Bingo, Socials, Black Jack, Happy Hour, etc. Redirection and Intervention * Under the direction of the Activities Director and within the scope of training provided: * De-escalates agitating behaviors by utilizing skills learned to mitigate the situation safely. * Performs physical intervention and redirection techniques (Mandt) when needed, utilizing the least restrictive measures available. Other related duties as assigned. CONDITIONS OF EMPLOYMENT: * Must be able to work a flexible schedule, day and evening shifts, weekends, and holidays and back-to-back shift and double shifts as needed. * Must be able to follow prescribed policies and procedures even if they may conflict with my "personal" preferences. * Ability to work with staff and patients of diversity to include social, economic, ethnic, and religious backgrounds in a long term setting * A Tuberculosis Test (TB) will be required upon hire. * A pre-employment criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions, conviction of crimes of moral turpitude, or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Additionally, you will be required take and pass a drug screen. * Former employees of the Colorado Department of Human Services (CDHS) who were disciplinary terminated or resigned in lieu of termination must disclose the information on your application and may not be eligible for employment at CDHS. * Must demonstrate the ability to lift 50 lbs, bend, squat, etc. and perform CAIT interventions proficiently (pre-employment and continuously throughout employment). * Position must travel to and from facility to provide outings for residents activities.
