Job DescriptionThe Colorado Coalition for the Homeless seeks a Clinical Case Manager to coordinate SUD and COD care for homeless clients seeking housing services and to support the knowledge and collaborative care between case management, housing, and treatment. This position will have an emphasis on relapse prevention, response, and support including the provision of individual and group care, crisis intervention, assessment, linkage to treatment services, and stability in housing This position is located in Denver, CO, and reports to the Residential Services Clinical Program Manager.NOTE: For compliance reasons, after a job offer has been extended, the candidate chosen to fill this clinical position will be required to complete our agency’s credentialing and privileging process prior to start date.   • Establish relationships with treatment providers, including understanding the scope of services available to CCH clients outside of CCH, such as inpatient treatment, recovery networks, and detoxification facilities.
  • Act as liaison between external services and internal services for SUD and COD as needed by the client and care team.
  • Provide crisis management to clients as needed.
  • Conducts biopsychosocial assessments and intake screening with appropriate diagnoses as deemed appropriate by the team.
  • Facilitates therapeutic meetings with clients in individual and group settings.
  • Facilitates substance abuse groups and individual therapy sessions as needed.
  • Provides ongoing clinical case management services to homeless persons with psychiatric and/or substance abuse diagnoses, in a varying degree of severity as needed.
  • Works collaboratively as part of a multi-disciplinary team, including working with case management to increase access to care for SUD and COD services as well as improve care coordination.
  • Communicates clinical assessments, interventions, and ongoing client contact through timely, thorough electronic record keeping, contact forms, and verbal communication.
  • Maintains accurate electronic health record and participates in audits and projects to help improve and maintain accurate records that meet standard of care.
  • Facilitates client connections with other service agencies, providing transportation and support as appropriate.
  • Works cooperatively to serve clients in a comprehensive, effective, and caring way.
  • Assists others with work that needs to be done at moments of urgency or crisis.
  • Serves as a liaison with collaborating agencies and other teams within the organization, establishing and maintaining effective working relationships with organizations and groups providing services to our target population. This job function requires availability by cell phone during regular business hours and off-hours as needed.
  • Navigates the health care benefits systems (i.e. Medicaid and Medicare) on behalf of clients.
  • Completes Medicaid authorizations and client database entries as well as any other reporting requirements necessary to obtain and sustain funding and provide services to individuals served in the program.
  • Monitors and maintains productivity required for position.
  • Meets formally with Program Manager and/or Director regularly for ongoing supervision.
  • Attends and participates in clinical and administrative meetings as deemed appropriate and necessary for the job.
  • Assist residents in problem-solving issues that may threaten their housing or recovery goals.
  • Serves as a liaison between client and property management staff when necessary to advocate for client’s housing stability.
  • Covers for other Clinical Case Managers as necessary at other CCH locations.
  • Performs other duties as assigned.   • Master’s degree in social work, counseling, or similar field; licensed LCSW or LPC strongly preferred. If unlicensed will be actively working towards completion of licensure requirements.
  • Certification for Addiction Counseling (CACII, CAC III, or LAC) required.
  • Experience and interest working with disenfranchised persons.
