    Registered Nurse Admissions- Part-Time

    Our Values: Positivity, Excellence, Collaboration, Openness, Support, Growth and Innovation POSITION SUMMARY:Provides initial information to patient/family regarding hospice services and determines hospice appropriateness; assists in admitting patients to hospice program. Facilitates admission process by providing medical records and other documentation to the physician and hospice staff. Acts as liaison with assigned hospitals, nursing homes, physicians and provides supportive care and education for patients and families.KEY REPORTING RELATIONSHIPS:

    • Reports to Admissions Manager or Team Lead.
    • Coordinates with Admissions Department, Medical Records, and physicians to determine eligibility for hospice care.


      • Consults with primary physician to determine eligibility for hospice care and gather most recent orders, medical history and physical exam.

        • Reviews data on prospective patients and performs initial RN assessment.

          • Meets with the patient/family and/or caregiver to explain services, determine clinical needs, place durable medical equipment, supplies, medications and oxygen orders as needed. Completes inventory of current medications and determines which medications relate to the terminal illness.

            • Completes documentation of the plan of care (POC) and initiates the clinical record process through electronic medical record (EMR).

              • Provides detailed information on all aspects of admission to the Hospice Team and Admissions Coordinator. Interacts with referring physician and other professional staff to ensure a successful admissions process.

                • Other duties and responsibilities as assigned.

                  CORE COMPETENCIES:

                  • Supports the Mission and Values of hospice.

                    • Knowledge of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of patients in a safe, secure environment.

                      • Assists patients, family members or other clients with concern and empathy; respects their confidentiality and communicates with them in a courteous and respectful manner.

                        • Communicates with hospice staff in a timely manner to facilitate understanding and care of the patient dynamics that may be pertinent.

                          • Demonstrates knowledge of the admissions protocols that pertain to all elements of the admissions process from the initial referral call to the mandatory primary care assessment.

                            • Collaborates with IDT members to ensure optimal patient/family care. Educates patient/family, staff and community about end of life issues. Actively participates in team/staff meetings, notifies IDT of any patient/family needs.

                              • Completes appropriate number of hours of hospice education annually as required by state regulations.

                                • Attends required nursing meetings and is responsible for information discussed.

                                  • Cooperates with all co-workers and demonstrates flexibility to perform other duties as assigned.

                                    • Maintains and respects professional and personal boundaries.

                                      POSITION QUALIFICATIONS:Minimum Education: Graduate of an accrediting school of nursing

                                      Minimum Experience: One or more years nursing experience (preferably in hospice or home care).

                                      Required License: Current RN license in state of Colorado. Valid Colorado state driver---s license and auto insurance.

                                      PHYSICAL REQUIREMENTS:

                                      • Ability to lift minimum of 50 lbs.
                                      • Lifts, positions, pushes, and/or transfers patients
                                      • Manual dexterity and mobility
                                      • Visual and auditory acuity

                                        WORKING CONDITIONS:

                                        • May be exposed to infections and contagious diseases
                                        • Regularly exposed to risk of blood borne diseases
                                        • Subject to varying and unpredictable situations
                                        • Occasional exposure to unpleasant patient or unit elements (sound, odor, sight, behavior)
                                        • May perform emergency care
                                        • Able to maintain job performance in emergency or crisis situation
