Position Summary

To provide clinical and administrative leadership and business discipline to a multidisciplinary staff. To ensure that the standard of care that is provided is appropriate to meet the needs of the assigned population. To facilitate personalized directed care and treatment.--

Education Requirements


  • Health related clinical degree

    Work Experience


    • 2 years home care/LTC experience, 1 year supervisory experience.

      License/Certification


      • Clinical License as related to degree- RN or PT or OT or MSW
      • CPR
      • Colorado Driver---s License

        Position Duties (essential functions, include % of time)

        QUALITY - 20%


        • Actively participates in quality improvement activities and promotes staff participation at all levels. Demonstrates ability to listen to and respond appropriately to staff and client concerns.
        • Maintains ongoing analysis of clinical service performance, monitors trends and initiates QI processes to further best practice and clinical excellence.
        • Collaborates well with all departments in reaching common goals.
        • Participates in and provides leadership to work with interdisciplinary teams associated with department process improvement activities and supervises the ongoing evaluative analysis of the newly developed processes in order to identify further improvements needed.
        • Evaluates, investigates and trends quality data to determine variances and implements action plans.
        • Act as a Diagnosis coding and OASIS documentation expert for education, competencies, outcomes analysis, trending and improvements for all supervisory, clinical and operational staff. Stays current with industry regulations and maintains a solid competency of the complexities of coding, HHRG computation and analysis and OASIS documentation, audits, trends and education as assigned.
        • Acts as Call Center software expert as assigned.
        • Knowledge of and able to coach, guide and review ICD coding as assigned.

          • Demonstrates critical thinking and decision-making skill in relation to multidisciplinary teams of clinical and operational matters.
          • Simple problem-solving to scientific inquiry and reflective judgment related to patient diagnosis, prognosis, care needs and realistic goals.
          • Knowledgeable resource advisor related to paradigms of care, care planning, discharge plans and goal achievement.
          • Excellent clinical intervention skills with demonstrated ability to educate multidisciplinary clinical supervisors and staff and others about topics essential to clinical excellence, skill development, optimal outcomes.

            COST - 20%


            • Assists in the development and oversights of annual operating budget. Demonstrates awareness of internal and external factors affecting compliance to budget and works creatively to reduce cost and improve
            • efficiency
            • Develops and tracks operations budgeted items against actual expenditures with acceptable cost ratio parameters.

            • Actively participates in developing budget model for clinical operations with revenue and expense projections.

            • Identifies and implements cost effective and process efficient strategies related to budget variances.

            • Demonstrates a thorough understanding of financial concepts and guidelines in managing a budget.

            • Evaluates, investigates and trends the financial data to determine variances to net revenue analysis and makes adjustments accordingly.

              PEOPLE - 25%


              • Participates in the selection, orientation and evaluation of staff. Provides coaching and mentoring as needed.
              • Identifies recruitment and retention opportunities for improvement and makes recommendations accordingly.

              • Establishes orientation schedule and pacing model related to new hire skill set and experience, with ongoing oversight to its success.
              • Leadership development of direct reports through modeling, coaching, mentoring, evaluation, constructive feedback, with written plan for ongoing and future development needs, to include timelines. Utilizes current leadership principles and techniques.
              • Mentors and coaches to success.

                GROWTH - 10%


                • Collaborates with agency management and multidisciplinary supervisors and clinical staff to develop and achieve agency annual goals and objectives
                • Evaluates, investigates and identifies ways in which the clinical department can contribute to reaching agency goals and objectives.
                • Actively participates in strategic planning sessions scheduled throughout the year.
                • Promotes growth and partnerships within the community.
