Job Description PACE Senior Primary Care & Medical Care Center -
Full Time - Hours: Monday - Friday 8 am - 4:30 pm - Employed - New
Graduates - Average Patients seen: 3-6 - Call Schedule: Minimal;
one week on-call every 3-4 months; No hospital call - Sign-On Bonus
- Compensation: $229,269.91 to $283,888.22 base salary - Benefits:

Physician and help older adults live independently while receiving
high-quality, all-inclusive care.What You'll Do- Serve as primary
care physician for PACE participants (55)- Provide preventative,
acute, and chronic care with a focus on aging with dignity-
Collaborate with the Interdisciplinary team to coordinate holistic
care plans. The interdisciplinary team includes on-site social
work, physical and occupational therapy, a registered dietician, a
day center coordinator, home care, transportation, and more.- Build
trusted relationships with participants and their
familiesRequirements:- MD/DO with interest in geriatrics- BE/BC in
IM, FM, or Geriatrics- Active CA license DEA- Meets 42 CFR Part
410.20- Minimum 1 year experience with frail/elderly population
