Job Description:
Job Description Essential Duties Connect and Communicate: Be the
communication hub! Keep nursing staff, physicians, pharmacists, and
family members in the loop about resident status—think of yourself
as the central nervous system of our care team. Listen and Act:
Tune in to resident, family, and physician concerns with genuine
care, then jump into action with solutions or escalate to the right
people. Your advocacy makes a real difference! Champion Care Plans:
Attend resident care conferences and keep care plans sharp,
accurate, and relevant—these are your roadmap to delivering
exceptional care. Master Medication Management: Prepare,
administer, and document medications like a pro, following
physician orders to the letter. Stay on top of inventory, ordering
new meds and retiring expired ones before they become a problem.
Document with Purpose: Keep detailed, accurate records of patient
care issues and report them through proper channels. Great
documentation = great care! Trust Your Instincts: Spot signs and
symptoms early, use your clinical judgment, and select the right
treatment or procedures. Your experience and intuition are
invaluable assets. Lead with Confidence: Supervise and mentor
