impacting the community! Responsibilities About the Role During
visits that can last up to one hour, our providers assess and
evaluate a member’s current health status, medical history,
medication adherence, social circumstances, and other potential
risks. This approach provides invaluable insights into a member’s
overall health that are often missed during standard office visits.
The Comprehensive Health Assessment (CHA) enhances care quality and
allows us to address multiple care gaps in a single visit. Our
providers are responsible for: Conducting assessments for adults
and geriatrics, including medical history, diagnosis, treatment,
health education, physician referrals, case management referrals,
follow-up, and thorough documentation in line with our protocols.
Administering and ordering necessary point-of-care and lab tests.
Collaborating with physicians, case managers, social workers,
family members, key caregivers, and other medical personnel as
needed. Working alongside Primary Care Physicians (PCPs) for
patient education and necessary follow-ups. Providing services at
the member’s residence, whether in their homes or Skilled Nursing
Facilities. Benefits of Working with Us Consistency: While
assessments are conducted from Sunday to Saturday between 8 AM and
7 PM, our full-time providers typically work Monday to Friday from
8:00 AM to 5:30 PM. Competitive Pay: Enjoy an annual base salary
ranging from $105,500 - $168,400 , determined by market analysis
and experience. Incentive Program: Participate in uncapped
quarterly incentive opportunities. Comprehensive Benefits: Includes

training, you will be assigned a preceptor, receive 1:1 feedback,
and gain free access to online continuing education. Additional
Licensure: If business needs arise, we will assist you in obtaining
additional state licensure and credentialing in neighboring states
or others you may wish to visit while conducting health
assessments. Qualifications Skills & Experience That Shine A
Master’s Degree or equivalent experience along with successful
completion of NP licensure. Current NP licensure in the state of
practice, including prescription authority or the ability to obtain
it as required. AANP, AACN, or ANCC board certification in Adult,
Adult Gerontology Primary Care, Adult Gerontology Acute Care,
Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or
CPR certification. A minimum of 1 year of NP experience.
Willingness to travel up to 80-90% of the time, including overnight
trips and weekends across the assigned state and possibly
neighboring states. A valid state driver's license and proof of
adequate automobile insurance for your state of residence. Strong
computer skills and familiarity with health/medical record
software. Excellent verbal and written communication abilities. The
