Our approach to primary care is distinct from the norm. As we continue to grow rapidly, we are looking for exceptional individuals to become part of our team. Job Overview Our organization, led by physicians and driven by a strong mission, is recognized as one of the leading full-risk Medicare Advantage providers in the country. We aspire to be the foremost primary care provider in America, dedicated to transforming the care of the most vulnerable populations. Our mission is to provide seniors with affordable, high-quality care that enhances their health. To fulfill this vision, we seek outstanding primary care providers who are eager to find purpose and personal growth within our family. Role of the Primary Care Physician (PCP) The PCP in our organization is expected to: Take Responsibility for Outcomes: The PCP is accountable for delivering strong clinical care and ensuring cost-effectiveness for a panel of up to 450 patients. Act as a Health Coach: Rather than merely addressing illness, the PCP helps patients establish and pursue both short- and long-term health goals, regularly following up to support their journey toward improved health. Simplify for Action: The PCP prioritizes tasks to make behavior change more achievable for patients and guides their care teams toward performance objectives. As a value-driven organization focused on outcomes, we regularly assess metrics such as patient admissions per thousand and patient experience scores to support PCPs in achieving success and partnership status. Each PCP will collaborate with center and market leadership to meet these targets, while also fostering a culture of love, accountability, and passion within our medical centers. The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will engage closely with managers and medical directors to ensure adherence to guidelines and participate in risk management and quality improvement initiatives. Engage with hospitalists for patients admitted to the hospital, regardless of their affiliation. Conduct assessments, diagnoses, treatments, management, education, health promotion, care coordination, and documentation for patients with complex health needs. Lead a care team that includes a medical assistant, care facilitator, and care coordinator for patients who can visit the office. Collaborate with transitional care teams for patients unable to visit the office, including those in hospitals, skilled nursing facilities, or homebound. Participate actively in the management of the center and assist in covering for absent providers, as well as in patient recruitment efforts. Supervise and collaborate with Advanced Practice Practitioners (APPs), remaining accountable for their actions. Perform additional duties as assigned by management. Knowledge, Skills & Abilities Essential Competencies: Availability and Accessibility: PCPs should be approachable and available for walk-in visits, responding to patient inquiries promptly to build trust. Service Orientation: Deliver care that reflects the same quality one would want for family members or themselves. Evidence-Based Medicine: Stay informed about evidence-based practices while recognizing the impact of lifestyle and nutrition on health outcomes. Physician Leadership: Continuously develop leadership skills to benefit patients, teams, and the organization. Quality Focus: Strive for the highest quality of care and work collaboratively with care teams to achieve excellent outcomes. Influence: Effectively and compassionately guide patients and teams toward optimal results. Self-Care: Prioritize personal well-being to ensure the best care for patients, encompassing physical, mental, social, and spiritual health.
