Job Description:

Seeking a compassionate and skilled Certified Nursing Assistant (CNA) to provide one-on-one care to seniors and individuals with chronic diseases in their homes.

As a Home Health Certified Nursing Assistant, you will provide personalized care to patients in the comfort of their own homes. This includes assisting with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake. You will also assist with ambulation, transfers and range of motion exercises, check vitals, measure intake/output, monitor blood glucose and document any changes. Additionally, you will work with a team of home health nurses and aides to ensure a patient's care plan is followed and observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment.
