SummaryUnder minimal supervision, provides medication therapy
management services and collaborates with other health care
providers to optimize therapeutic outcomes for Denver Health
patients. Develops policies and procedures, clinical guidelines, or
other documents required to operate the service. Plays active role
in pharmacy department quality initiatives and staff education.
Responsibilities
- Designs therapeutic drug regimens or plans based on
patient-specific information that includes establishing a
therapeutic endpoint, selecting appropriate drug and related
therapy, and establishing an appropriate dosing regimen.
- Demonstrates sound problem solving/decision making skills with
practical, efficient, economical, and reasonable solutions.
- Monitors medication therapy for potential drug-drug, drug-food,
or drug-disease state interactions.
- Assess and monitor patient response to medication
regimens.
- Documents all patient activities and interventions in the
patient's medical record or pharmacy computer system. Demonstrate
accuracy, thoroughness, and attention to detail.
- Establishes a presence and working relationship with nursing
staff and providers involved in the care of Denver Health
patients.
- Collaborates with providers in establishing treatment plans for
patients.
- Provides assistance in the following areas: formulary or
preferred agents, evidence-based medicine, drug information to both
providers and patients, drug interaction screening, medication
profile review, assessment of patient response to therapy.
- Provides medication counseling for patients.
- Identifies and supports clinical and/or operational quality
improvement opportunities.
- Develops new and/or existing reviews, guidelines, and protocols
relevant to area of expertise.
- Reports and follows up on medication/patient safety issues as
needed; works to resolve problems in a manner that will improve
patient safety outcomes and clinic efficiency.
- Understands patient safety goals, core measures, and other
quality metrics that affect area of practice; assists in
development of programs that ensure compliance with these
measures.
- Participates in and provides educational programs for the care
team members. Assists in the development of competency modules for
staff that pertain to clinical specialty area.
- Provides education and training: serves as a preceptor for
pharmacy residents, pharmacy interns, pharmacy students on rotation
at Denver Health. Participates in the training of new
pharmacists.
- Attends and participates in departmental and/or system-wide
meetings, in-services, huddles, and committees. Completes
self-assessment of current practice goals and long term goals
annually. Identifies opportunities for learning and training to
advance own professional competence.
- Ensures that all legal and professional pharmacy requirements
are adhered to. Maintains compliance with Joint Commission and/or
other government regulatory standards applicable to area of
practice. Maintains an active pharmacist license in good standing
to practice in the State of Colorado. Maintains continuing
education requirements for licensure.
- Promotes positive interpersonal (customer) relationships with
fellow employees, physicians, patients and visitors. Treats these
individuals with courtesy, dignity, empathy and respect;
consistently displays courteous and respectful verbal and
non-verbal communications.
- Adheres to, complies with and demonstrates support for the

- Ensures confidentiality of patient information by creating and
maintaining a secure and trusting environment by not sharing
information learned on the job, except when necessary in the
performance of the job responsibilities or to improve a patient's
care.
- Has regular and predictable attendance.
Knowledge, Skills & Abilities* Broad
pharmacotherapy knowledge for managing patient medication therapy
needs, including current treatment guidelines and literature.
* Strong communication and active listening skills.
* Ability to provide input into the vision and development of
clinical pharmacy services.
Computers and Technology
* Proficient in Microsoft Office Suite.
Education * Bachelor's Degree (Required) One year of experience in
the provision of medication therapy management services
required.
* Doctorate Degree Experience
- One year of experience in the provision of medication therapy
management services required.
Credentials * Licensed Pharmacist (Required)
