Job Description:
Job SummaryUnder general supervision, assists in developing and
implementing comprehensive diabetes patient education program. In
collaboration with unit based leadership, the CNE assists with
patient and family education and staff training. Assists with
ongoing program assessments and assures patients and families
receive education that is evidence based and meets community and
national standards. Additionally, the CNE participates in the
coordination and facilitation of interdisciplinary patient centered
diabetes care across the continuum. ResponsibilitiesKnowledge,
Skills & AbilitiesEducation: Bachelor's Degree in Nursing required.
Master's degree preferred. Certified Diabetes Educator (CDE)
certification preferred, will consider CDE eligible. Experience:
Typically, two years of professional nursing experience, including
current, practical experience as a diabetes educator. Strong skills
in project design and implementation is preferred. Knowledge,
Skills & Abilities: Bilingual (Spanish) preferred. Knowledge of
staff education and professional nursing principles, practices,
concepts, and procedures. Has knowledge of relevant educational
content and appropriate delivery methods. Ability to develop and
facilitate relevant and effective education for adult learners.
Knowledge of standards, polices and procedures of the agency.
Knowledge and utilization of a variety of educational modalities.
Knowledge of performance improvement. Skill in the development,
implementation and evaluation of education programs, competency,
policies, procedures and standards of care. Ability to identify
learning needs and assist with meeting those needs. Ability to
collaborate with members of the multidisciplinary team and
colleagues in the Ambulatory Care setting in assessing, planning,
implementing and evaluating diabetes education
programs/competencies to facilitate safe patient care. Ability to
identify and collaborate with content experts to develop activities
to facilitate learners' achievement of the educational objectives.
Ability to use computers and computer systems. -Active Registered
Nurse (RN) License in the Sate of Colorado or a participating
compact state. Basic Life Support for Health Care Provider
Certification/CPR. Certified Diabetes Educator (CDE) preferred or
must be eligible to apply for certification. All Certifications and
Licenses required for this job must be kept current as a condition
of continued employment. Shift DaysWork TypeFull time Pay
