Job DescriptionThe Colorado Coalition for the Homeless seeks a Clinical Case Manager to provides behavioral health case management and individual therapy to single adults who are homeless and who have a severe and persistent mental illness in keeping with the goals of the PATH program and in accordance with the overall mission, values, and objectives of the Coalition. This position is located at 2100 Broadway Street Denver, CO 80205 and reports to the PATH Program Manager.NOTE: For compliance reasons, after a job offer has been extended, the candidate chosen to fill this clinical position will be required to complete our agency’s credentialing and privileging process prior to start date. Essential Job FunctionsProvide behavioral health case management, individual psychotherapy, and related clinical services to assist clients in accessing resources and developing personal skills to:
  • Secure and maintain stable housing;
  • Engage/re-engage/remain engaged with healthcare services to improve and/or maintain overall health and well-being;
  • Establish positive and supportive social connections in the community;
  • Identify and pursue meaningful activities that provide a sense of purpose and personal satisfaction.Specific Job Duties
  • Conducts screenings and assessments including diagnostic evaluations and determines medical necessity for behavioral health services.
  • Provides behavioral health case management to assist clients in accessing and maintaining all social service benefits to which they are entitled, including but not limited to subsidized housing, SSA benefits, vocational and educational services, etc.
  • Provides counseling services, as appropriate, using treatment models including motivational interviewing, solution-focused therapy, and cognitive behavioral therapy. Provides targeted brief interventions and therapy.
  • Provides crisis de-escalation and crisis management services, including careful, thorough risk assessment coupled with appropriate interventions and follow through.
  • Coordinates care with the consulting psychiatric prescribers, primary care physicians and behavioral health professionals in making diagnoses, formulating treatment plans, and monitoring client progress.
  • Serves as liaison with external agencies involved with client care to establish and maintain effective working relationships on behalf of clients.
  • Provides individualized and enhanced coordination of care across the treatment spectrum with goals of maximizing functioning within the community.
  • Works with clients to identify interests and links them with agencies and groups to facilitate engagement with meaningful activities.
  • Demonstrates respect, cultural competence and knowledge of trauma-informed care in interactions with clients, staff, and external community providers.
  • Provides transportation in personal vehicle or CCH vehicle or arranges for other transportation assistance to help clients in accessing care and meeting treatment goals.
  • Assists patients in accessing basic needs directly related to their behavioral health care needs such as shelter, food, obtaining prescriptions, etc.
  • Maintains on-going competency in the use of HMIS, which includes accurate and timely documentation and careful tracking of all data elements required by the PATH grant.
  • Participates in peer review and peer clinical audits and evidence-based practice projects as requested.
  • Makes appropriate referrals to outside entities, including specialists and others, to provide additional supports, whenever indicated, especially for the care of clients with complex or severe mental health, substance use and/or medical concerns.
  • Attends meetings on time and participates in clinical and administrative meetings as deemed appropriate and necessary for the job.
  • Participates in regular clinical and nonclinical supervision.

  • Performs additional duties deemed necessary and appropriate by the PATH program manager and/or the DOES director.
  • Maintains on-going competency in the use of the EHR which includes:
    • Recording all patient visits in a timely and complete fashion.
    • Completing all documentation within 24 hours of clinical contact.
    • Maintaining up to date records in ELF on all current and active clinical clients.
    • Recording telephone and other clinical communications as needed in the EHR.
    • Managing tasks, PAQ and scanning as needed to have a complete and current chart.
    • Maintaining accurate information regarding the client’s insurance status and complete Medicaid authorizations, CCARs, DACODES, and other clinical authorization documentation as needed to sustain funding and provide services to individuals.
    • Utilizing the EHR and any future upgrades as directed

    • Master of Social Work, Counseling, Psychology or related degree required.
    • Licensure as an LCSW /LPC or in the process of working towards licensure required.
    • At least one year of experience serving people who are homeless and/or impoverished and who experience behavioral health challenges required.
    • Clinical experience interacting with clients in integrated health care, mental health care, or substance use treatment settings preferred.
