Case Manager, your mission is to provide more than care; it's to
provide comfort, understanding, and compassion. You will:Offer
direct patient care, tailored to each individual's unique

the evolving requirements of patients and families.Collaborate
seamlessly with a diverse healthcare team to deliver comprehensive
hospice services.Be the bridge between patients, families, and

hospice care.Your Qualifications:To excel as a Hospice Case
Manager, you'll need:A current and unrestricted Registered Nurse
(RN) license.At least three years of nursing experience, with a
preference for hospice, palliative care, or home health
experience.Proven case management skills, with a knack for
orchestrating patient care.Stellar communication and interpersonal
skills, essential for providing compassionate support.Strong
organizational prowess to handle a demanding caseload.Deep
understanding of end-of-life care and ethical considerations.The
ability to shine independently and thrive within a
multidisciplinary team.Basic computer skills, including proficiency
