Job Description:
Job Description Summary: Assists with the daily coordination of
personnel and resources for case management services. Work
Schedule: Full Time Days This is a special projects RN Care Manager
Lead. Responsibilities include special projects development and
implementation for the palliative care service, including
value-based initiatives, advance care planning navigation for
patients living with cancer, and helping to develop and implement
palliative care-related quality programs/initiatives in the
outpatient and home setting. Responsibilities: Coordinates staff
scheduling, educational/development activities, and performance
evaluations. Identifies and resolves issues affecting the delivery
of clinical services. Performs quality assurance studies and
assists with implementation of QA/QI initiatives. Serves as a
resource to staff and external contacts on issues related to
patient care, patient flow and clinical standards, policies and
procedures. Monitors departmental compliance with applicable
requirements, including accreditation, legal, and The Joint
Commission. Within scope of job, requires critical thinking skills,
decisive judgement and the ability to work with minimal
supervision. Must be able to work in a fast-paced environment and

Registered Nurse Other information: Minimum Required Education:
Bachelor's degree in Nursing. Required Licensure/Certification:
State licensure as a Registered Nurse (RN). Preferred: Basic Life
Support (BLS) Healthcare Provider. Minimum Experience: 2 years of
experience as a case manager or as an RN in the assigned clinical
area, with demonstrated clinical competence. IF APPLICABLE: BLS
through the American Heart Association or the American Red Cross
CPR for the Professional Rescuer with card in-hand before start
date. BLS or CPR card must be good through sixty days of hire. All
life support certification cards must be issued by either the
American Heart Association or the American Red Cross CPR for the
Professional Rescuer. (American Red Cross for the Professional
Rescuer is only good upon hire and all renewals will need to be
through an American Heart Association sanctioned course.) ---

pay range for this position is: $38.29 - $59.35 / hour. Pay is
dependent on applicant's relevant experience. UCHealth offers a
