Job SummaryThis unique role utilizes nursing skills in a mostly non-clinical manner. The position supports the Nurse Family Wellness program. The Nurse Family Wellness program provides home visitation and case management for families referred from Denver Human Services. Under minimal supervision this position facilitates, coordinates, and directs medical care, resources, and education for families. The RN participates in appropriate team decision making meetings and other interdisciplinary care rounds to review treatment goals, progress, and identified needs. The nurse is responsible for a caseload of 24 families.ResponsibilitiesEssential Duties and Responsibilities: 1.Facilitates effective and appropriate use of healthcare resources with clients. 2.Identify community resources which can aid with needed health supplies and equipment. 3.Participate as appropriate in team decision making meetings and other interdisciplinary care rounds to review treatment goals, progress and identified needs.4.Collaborates with other members of the healthcare team and specialists as needed. 5.Collect and review in-depth health psychosocial information about the child in order to develop comprehensive case management plan to meet the child's unique needs. 6.Foster an environment for open communication and collaborative practice. 7.Appropriately documents all patient care including consultation with other healthcare providers and specialists. 8.Data is accurate, complete, and entered in the appropriate case management system. 9.Includes communication to external providers and/or agencies when necessary.10.Participates in Red Team and CPT meetings with Denver Human Services.11.The Nurse Support Program provides nurse home visitation services to families referred by Denver County Department of Human Services. 12.The Nurse Support Program is a home visitation program that is designed to support and help families stay together. 13.The nurse assists families with health and parenting goals through education, support and resources, with the goal of self-sufficiency and to reduce further county involvement. 14.The focus of the home visits is based on the needs of each family. 15.The nurse is responsible for a caseload of 24 families, meeting program goals and objectives, as well as client-centered goals, health assessments and interventions.16.Initiates and participates in any other duties and tasks needed to help drive Denver Health's vision, fulfill the organization's mission, and abide by our organization's values.17.Other duties as assigned. Knowledge, Skills & AbilitiesMinimum Qualifications Education and Experience 1.Bachelor's of Science Degree in Nursing required. 2.One year of RN Registered Nurse experience preferred. Knowledge, Skills and Abilities1.Demonstrate ability to work with DHS agency employees, the general public, and children and parents in the community. 2.May involve interaction with educationally, culturally and racially diverse persons whose first language may not be English.3.Demonstrate an understanding of assessment findings as they relate to disease process and continuum of care. 4.Ability to assess patient's needs and coordinate proper nursing care. 5.Ability to exercise initiative and judgment in selecting proper treatment.Computers and Technology1.Proficient in Microsoft Office Suite.2.Health Information Management experience preferred.Certificates, Licenses and Registration1.Active Colorado RN license required. 2.Basic Life Support (BLS) certification required.
